PRIVACY POLICY

Movepod Podiatry is bound by the Privacy Act 1988 (Cth) and the Health Records Act 2001 (Vic), and its practitioners operate within the Code of Conduct, guidelines and standards of the Australian Health Practitioner Regulation Agency (APRHA). We require your consent to collect personal information about you. Please read this information carefully, and sign where indicated below. Upwell Health Collective collects information from you for the primary purpose of providing quality health care. We require you to provide us with your personal details and a full medical history so that we may properly assess, diagnose and treat injuries and conditions and be pro-active in your health care. We will also use the information you provide in the following ways:
• Administrative purposes in running our health practice
• Billing purposes, including compliance with Medicare and Health Insurance Commission requirements
• Disclosure to others involved in your health care, including treating doctors and practitioners internally and externally to Upwell Health Collective. This may occur through referral to other doctors, or for medical tests and in the reports or results returned to us following the referrals. If necessary, we will discuss this with you.
• Disclosure for research and quality assurance activities to improve Upwell Health Collective’s model of care and practice management.
CANCELLATION POLICY

We know that life gets in the way sometimes and we ask that you give us at least 24 hours notice when cancelling and appointment. This allows us enough time to offer your appointment time to another client who is trying to stay healthy and active. If we don't receive 24 hours notice we will be required to charge 100% of the consult fee as we will be unable to fill you appointment slot on such late notice. This same policy applies to any missed appointments.
PAYMENTS

At Movepod Podiatry we are a fee for service business which requires settlement of accounts on the day the appointment. If you have a compensable claim which is still pending, it will be your responsibility to settle accounts on day of appointment until your 3rd part provider accepts your claim.
PARENT OR GUARDIAN
Patients under the age of 18 must have their New Patient Form filled in a signed by a parent or guardian. Parent or guardian must agree to the policies and understand the reasons why personal information must be collected. Guardian must also be aware that Movepod has a privacy policy on handling patient information. I understand that I am not obliged to provide any information requested of me, but that my failure to do so might compromise the quality of the health care and treatment given to the patient. ​
